Consists of Drs. Sanjeev Goel, Lopita Banerjee, Manisha Sharma and Ken Kruger along with Melissa Dore (RN) EC.  Primary health care includes but is not limited to: disease management and prevention, disease cure, rehabilitation, palliative care and health promotion.
We are pleased to share the news the Ministry of Health has awarded the Wise Elephant team status as a Family Health Team (FHT), an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for our patients. FHTs are designed to give doctors support from other complementary professionals, including other specialists, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills to ensure your healthcare needs are met.


Getting seen faster Although your family doctor will remain the same, you may now be seen by any member of our team, reducing the time it takes to get an appointment. Members of our FHT share patient charts; as a result, all providers have immediate access to your information in order to best help you. Your doctor will receive documentation about the nature of your problem so that they will always be up-to date about your medical status.
More hours of service. Broadening our team means you’ll have access to a Wise Elephant practitioner during extended evening and weekend hours for urgent problems.
Access to additional services Based on the needs of our community, you will receive improved access to services such as counseling, diabetes management, prenatal care, dieticians and nutritional counseling and other forms of care that will improve your health.
Telemedicine If you need assistance after-hours, you will be able to telephone the Family Health Team Telephone Health Advisory Service where a registered nurse will provide advice about your urgent health care concerns.
